Affinity Program Application

Please complete the following 2 steps:

Step 1 - Fill out the Cognigen Business Opportunity registration form This step will create your account and provide the important instructions about your future website.

Login your private website After you complete the first step, please take a moment to view your admin site where you can find useful information and tools. We recommend you access the "edit your profile" page where you can put your own logo on your new website and turn off the "business opportunity" link (if necessary).

That's all you need to do to get started!

Additional Options Available

After you have completed the steps above, you are ready to go. In you operate a large group and have additional requirements, the following optional services are available:

Custom logo design
We will design the logo per your instructions for $25 (one time fee).

Custom logo design with hosting
We will design the logo per your instructions and host it on our server for $45 (one time fee).

Custom built website
We will customize the website per your instructions. The price is dependent on the size and complexity of the project, and will be determined after your specific requirements are reviewed.
